The PACE Story

The inspiration behind PACE began with a deeply personal experience.

Like many family caregivers, Tiffany Matthews found herself navigating a role she never expected to assume.

When her grandmother, Elnora, entered the final chapter of her life, Tiffany became more than a granddaughter.

After many years in social work and healthcare, she realized she was an advocate for patients. Now, she had to help her Gram, who really was a mother to her.

What began as helping a beloved family member quickly became a journey through a healthcare system that often felt overwhelming and difficult to navigate. Like many families, Tiffany and her loved ones did not know where to turn for answers, resources, or guidance. They were faced with unfamiliar medical terminology, complex decisions, care transitions, and questions that seemed to multiply with every appointment.

As she worked alongside healthcare professionals, including a compassionate healthcare social worker, Tiffany discovered something unexpected about herself. During her Gram’s end-of-life, she learned how important advocacy could be for patients and families trying to navigate healthcare challenges during some of the most vulnerable moments of their lives.

As she helped others navigate appointments, care transitions, healthcare systems, difficult decisions, and everyday caregiving responsibilities, she gained firsthand insight into the challenges that millions of families face every day.

She also witnessed the emotional, physical, and financial strain that caregiving places on families. The experience revealed how often caregivers are expected to manage complex responsibilities without preparation, support, or recognition.

The impact was especially visible within her own family. As her mother’s caregiving responsibilities intensified while trying to maintain work obligations, the weight became overwhelming. Tiffany watched her mother struggle under the competing demands of caregiving and employment until she ultimately collapsed under the pressure. That moment underscored a reality that countless families experience every day: caregiving does not exist separately from work, health, or wellbeing. The effects are deeply interconnected.

When Elnora passed away, Tiffany carried with her not only grief, but also a sense of purpose.

She made a promise. She vowed that the lessons learned through her grandmother’s journey would not end there.

She committed herself to helping others better navigate healthcare and caregiving so that families would feel less alone, more informed, and better supported during life’s most challenging transitions.

She also began to notice something larger.

The impact of caregiving did not remain at home.

It followed people into workplaces. Every day.

It influenced professional performance, emotional wellbeing, healthcare experiences, family dynamics, and daily decision-making.

Yet these realities often remained invisible to the organizations, systems, and institutions surrounding them.

That realization became the foundation for PACE, which is the home of The Caregiver Effect™ and Elnora’s Place™, a platform to help caregivers & those they care for navigate healthcare, improve patient care and advocate effectively. The mission is to help organizations gain stability with their workforces through PACE.
