It’s designed to empower caregivers to navigate healthcare, advocate effectively and improve patient care for the care recipient
Named after my beloved Gram, Elnora Farmer, I created a community just like she ran her home – with love, wisdom and support. She’d also tell you a thing or two about a little of everything.
After watching Gram suffer a terrible end-of-life experience in the healthcare system and our family being crushed by the weight of caregiving, I knew I had to help not only patients, but caregivers and families with healthcare, patient care & caregiving. This is how.
Elnora’s Place
Where caregiving becomes visible, navigable, and human again.
Elnora’s Place exists because the healthcare system is very good at treating conditions—and remarkably bad at caring for people and the families who love them.
This is not a theoretical initiative. It’s not a passion project dreamed up in a boardroom. Elnora’s Place was born in real hospital rooms, late-night phone calls, impossible decisions, and a caregiving burden that nearly broke an entire family.
It is named for my grandmother, Elnora—and it carries her legacy forward with clarity, dignity, and purpose.
The Origin Story (The One No One Wants, But Millions Live)
At the end of her life, my grandmother experienced what far too many older adults experience in America: fragmented care, confusing systems, conflicting medical advice, and a complete lack of coordination between providers.
There were specialists who didn’t talk to each other. Instructions that contradicted one another. Appointments scheduled with no regard for physical limits or cognitive decline. Decisions that were framed as “choices” but offered without real explanation, context, or support.
The system did what it does best—it optimized for billing, throughput, and silos.
What it did not do was care for the whole person.
And while my grandmother suffered through this maze, my mother carried the invisible load of caregiving.
She managed appointments. Medications. Paperwork. Transportation. Emotional labor. Advocacy. Fear. Guilt. Exhaustion.
Day by day, the weight compounded.
Eventually, the caregiver broke.
Not because she didn’t love enough.
Not because she wasn’t capable.
But because no one is meant to carry this alone—and the system quietly assumes families will.
That breaking point is where Elnora’s Place was born.
What I Saw—and What I Couldn’t Unsee
I watched a woman I loved deteriorate inside a system that treated her as a set of diagnoses instead of a human being.
I watched a caregiver I loved absorb risk, responsibility, and emotional strain that was never acknowledged, supported, or measured.
And I realized something that changed the trajectory of my work forever:
Caregiving isn’t invisible because it’s rare.
It’s invisible because systems are designed not to see it.
Elnora’s Place exists to correct that.
What Elnora’s Place Is
Elnora’s Place is a caregiver-centered advocacy and navigation model rooted in lived experience and operational reality.
It is the foundation of everything I do.
At its core, Elnora’s Place:
Centers the caregiver as a critical part of the care team
Translates complex medical and healthcare systems into plain language
Supports decision-making without judgment or pressure
Advocates for dignity at every stage of care, including end of life
Makes the invisible labor of caregiving visible—and valued
This work predates frameworks, pricing, and programs.
It came first.
From Family Experience to Scalable Impact
What began as a response to one family’s crisis evolved into a broader realization: millions of families are navigating the same chaos every day.
Caregivers are expected to:
Coordinate care without training
Navigate healthcare systems without a map
Advocate without authority
Grieve while still performing
And when caregiving happens alongside employment, the strain multiplies.
That’s where Elnora’s Place connects directly to my enterprise work through PACE (Patient Advocacy & Caregiver Empowerment).
Elnora’s Place is the why.
PACE is the how.
Together, they translate caregiving reality into systems leaders can understand—and act on.
Why This Matters to Employers and Institutions
The story of Elnora’s Place is deeply personal—but its implications are organizational.
Caregiving shows up in the workplace as:
Absenteeism
Presenteeism
Burnout
Turnover
Disengagement
Quiet exits
Most organizations don’t fail caregivers intentionally.
They simply don’t see them.
Elnora’s Place ensures that caregiving is no longer treated as a private problem with public consequences.
My Role
I’m Tiffany Matthews—patient advocate, caregiver strategist, and your Healthcare BFF.
I don’t approach this work as an outsider or a theorist.
I approach it as someone who has:
Lived caregiving at end of life
Witnessed system failure up close
Supported caregivers at their breaking points
Built structured, data-driven frameworks to prevent that breaking
Elnora’s Place is not about reliving trauma.
It’s about preventing the next family from being crushed by it.
The Legacy
Elnora’s Place stands for something simple and radical:
Caregivers deserve clarity.
Families deserve dignity.
Systems must do better.
This work honors my grandmother by refusing to accept fragmentation as normal—and by ensuring that no caregiver is ever again expected to hold the entire system together alone.
That is the promise of Elnora’s Place.
And it’s the foundation beneath everything that follows.
Why Elnora’s Place Matters to Employers and Health Systems
Elnora’s Place is not just a family story. It is a systems case study—one that mirrors what employers and healthcare organizations are experiencing at scale every single day.
What happened to my grandmother did not happen because people didn’t care. It happened because:
Care was fragmented across providers and settings
No one owned coordination or accountability
Families were forced to become unpaid care managers overnight
Employers and health systems absorbed the downstream cost—quietly and repeatedly
That same dynamic is now showing up in workforce data, utilization trends, and employee engagement metrics.
What Employers See (Whether They Name It or Not)
Elnora’s Place reflects the reality inside today’s workforce:
Employees managing complex care while working full-time
High performers burning out quietly
Increased absenteeism and presenteeism tied to caregiving strain
Rising healthcare spend without corresponding improvements in outcomes
Most employers address this reactively—through EAPs, wellness apps, or one-off benefits that employees don’t use because they don’t solve coordination problems.
Elnora’s Place exists because caregiving strain is not a motivation issue—it is a systems issue.
What Health Systems Recognize Immediately
Health systems see Elnora’s Place as familiar territory:
Delayed discharges due to caregiver overwhelm
Readmissions driven by poor home support
Families confused by treatment plans, medications, and follow-up care
Clinicians doing their best inside broken handoffs
Elnora’s Place demonstrates what happens when care navigation, advocacy, and human support are missing—and why outcomes suffer even when clinical care is strong.
From Lived Experience to Enterprise Strategy
Elnora’s Place is where insight was forged. PACE is where it became actionable.
The experience revealed a clear truth:
You cannot improve outcomes, retention, or cost without addressing the invisible labor families and employees are already performing.
That truth now informs how I work with:
Employers designing caregiver and workforce strategies
Health systems focused on transitions of care and patient experience
Universities supporting faculty, staff, and clinical teams
Elnora’s Place ensures this work stays grounded in reality—not theory, trends, or surface-level wellness initiatives.
Why Organizations Partner With Me
Organizations that engage with my work are not looking for performative benefits or another vendor.
They are looking to:
Understand where strain actually lives in their system
Quantify the cost of unmanaged caregiving and health complexity
Design support that employees and patients will actually use
Align workforce strategy with healthcare realities
Elnora’s Place is the reminder—and the proof—that when systems fail to support caregivers, everyone pays.
And when organizations lead with clarity, coordination, and compassion, the return is measurable—for people and performance alike.
A Corporate Solution for Your People...
Our Vision:
To transform how employees—and the loved ones they care for—experience caregiving by empowering them with expert guidance, compassionate community, and life-changing advocacy tools. Caregivers shouldn’t have to choose between their job and the health of someone they love. This helps them do both, better.
Who It Helps:
Family caregivers of aging parents, children with medical needs, or loved ones with chronic illness
Employees who are overwhelmed by medical appointments, insurance issues, or long-term care decisions
HR leaders seeking meaningful, measurable benefits to reduce turnover, absenteeism, and burnout
Why It Matters:
Caregiving isn’t just a personal issue—it’s a workplace issue. Over 1 in 6 employees is a caregiver, and many don’t speak up until they’re forced to miss work, underperform, or resign. This community helps them feel seen, supported, and empowered with actionable help. When caregivers thrive, everyone benefits.
