Safe Discharge Support - $250

This package has 2 hours of support, an instructional checklist and 7-day follow up.

Discharge & Home Support - $450

This package has four hours of support, I will speak to the facility with you to assure the process goes smoothly and provide instructions and follow-up in 7 and 30 days.
